I have no fluff to bring (pa-rup a pup pom!) but am dying for suggestions and figured that this is the place.
If anyone out there loved the film Children of Men as much as I, and are aware of the Scifi channel series that is being developed based on the film, wouldn't the story line provide a great Cities of Death scenario?
Wish I had more to offer here, but it is a little after 30a.m. and I am on prescription medication so though the idea came, nothing came with it. But it's a great idea, I think.

How do you mean? I fail to see how a babyless world would make a great CoD scenario. Please elaborate on it, since I too loved the movie!
I think he was just inspired by the badass cityfight scene in the movie, and all the poverty and half ass buildings....very Imperium

I have encorperated Children of Men into my terrain. I have no babies in my terrain.
I saw a guy on another forum who painted his Guardsmen and converted them so they'd look like the British Army from the climactic battle of that movie. They look awesome. Here are some links. I loved Children of Men, haha.
After seeing these, and reading the guy's guide on how easy it is to paint models like those fellows, it made me wish I didn't have 100+ guardsmen painted as I do haha!
Children of Men is a great film. The reason why you're thinking it would be great for 40k is because it's exactly 40k material - dystopian, fascist, worn-down and violent.
Incorporating the idea of worldwide sterility into army fluff or even fanfiction is something I haven't seen before. It's a very novel idea, though for it to work you'd need one of two things:
- for the planet to be cut off from outside influence, or else the Imperium would just migrate some fertile people in.
- for the effect to be constant; like a machine that suppresses conception on a worldwide scale.
Otherwise, the crisis wouldn't be as critical. People could just pack up and fly to another planet where people can have babies.
As for who's doing it, well...we can always blame the Eldar...

Great idea, and basing it on 1 of the best movies from the last decade isn't bad either!
I agree, it would be a novel idea, it would certainly lead to a lot of unrest, and i too have looked at the cityfight scenes and gone "that would make a great wargaming table".
Let us know where you take this idea!
