I'm a little confused as to what technologies are truly lost to the imperium/AM. Some of the novels I've read indicate that the admech continues to crank out new starships and titans, others indicate that some of the older and more effective builds can no longer be replicated.
Can they just make warhounds and reavers, but not imperators? I'm also unsure about terminator armor and dreadnaughts. Can new suits of terminator armor and dreads be made (albeit with great difficulty) or has that been lost? If it has been lost, then how would the fluff justify a successor chapter having any TDA or dreads at all?
Both are correct. Some technology was lost and can no longer be replicated, but Adeptus Mechanicus continues to build ships and weaponry with the templates they still posess.admech continues to crank out new starships and titans, others indicate that some of the older and more effective builds can no longer be replicated.
Also there is the matter of substituting non-replicable components with cheaper and still producable equivalents (vide Baneblades).

The AdMech ritualise the creation of machines, and as a result, a lot of the important parts can be slowly forgotten or become misunderstood.
They can crank out Titans but they might not be as effective as the older models because they have forgotten about the lugnut #5239 which allows the reactor coolant to work at peak efficiency.
There are AdMech researchers and scientists and reverse engineers too, and they develop new techniques and actually advance the knowledge, but as they are so jealous of information, and because the Imperium is so immense. Any advances are very slow to trickle down to the Fighting arms of the Imperium.

Also, in many cases, they simply don't replace the vehicles because they deem they have 'enough'. The battleships, for example, are so massive and powerful that they're very rarely destroyed. And, of course, the newer battleships pale in comparison to the old-tech ones, which is why they prefer to salvage these ships from space hulks.
Lastly, the AdMech worship machinery. Pretty much anything holier than a pencil is blessed through a series of rituals. Now, imagine a Titan, and all its components (and the systems those components fit into, the pieces of equipment, etc), and assume that each piece 'only' requires a day to 'bless'. That prolongs the production rate considerably.
If the AdMech ever shifted to assembly lines instead of half-mystic hoodoo, then the Imperium would quickly be able to fully arm its forces a lot better (IG numbers with Space Marine equipment--Space Marines all running around in tactical dreadnought armor!). But, of course, they won't.
There have been several tech advances throughout the year ('specially after the Heresy). Usually, though, any changes in a tank or weapon mix results from the AdMech modifying existing hulls.
EDIT: In older fluff, they could not replace Terminator armor, so when the person who died, a teleportation beacon would activate and retrieve the armor for repairs.

Yeah. Plasma weapons and Land Speeders are pretty much the only new stuff that's been developed post-herersy. Variants on existing designs aside.
And the Assault Cannon too, but HH Novels put it under development at their time. So that might be a moot point.
Yeah calling all technology sacred and the purview of some holy order really killed the desire to tinker to find a better way to make something work.
It is one area where I like the Tau. They see technology as a means to an end and keep improving existing designs.
Remember that it's Black Library. It's not canon, and any actual fluff overrules it.
The thing that's been hammered in over and over again is that Zealotry and mystisism are keeping the AdMech from really developing anything.
