Nope. Chapters dont merge, not even from the same geneseed.
If they are good vs xeno races they can merge with ordo xenos. I think one of the almost destroyed chapters who fight against kraken tyranid hivefleet have merged with ordo xenos, but not 100% sure.
Both the Lamenters and the Scythes of the Emperor got nearly wiped out by the Hive Fleets (I think Behemoth was mainly responsible for that), and the latter is only ever mentioned in context to the Nids. That would be the one you're looking for, though I haven't heard anything about them merging with the Ordo Xenos.
I' read a bit of fluff once that mentioned remnants of a nearly destroyed chapter being assigned to help train newly founded chapters, but I'm not sure if they where assimilated into that chapter or not. I'd imagine the marines themselves would be very reluctant to do something like that, a Space Marine's chapter is what he lives for. He wouldn't want to abandone the traditions and methods of his chapter to join a new one.

If the two Chapters were sufficently similar in philosophy and doctrine and if their geneseed was compatible, than it is conceivable that they could organise a merger during times of desperate need. Marines whose Chapters have had their geneseed banks destroyed are often forced to be shifted to other Chapters while their own army is rebuilt from scratch.
Chapters die off all of the time. There are multiple instances in GW fluff books indicating that chapters become extinct or are wiped out, and new ones created. I have never heard of 2 chapters joining together, although I am sure that there is some teaching of new chapters by the commanders of another. I suspect that it makes the most sense for commanders of the chapter whose geneseed is used to be involved in the training of the initial recruits for a new chapter, and possibly to lead that new chapter as well (I think here of Sigismund, formerly of the Imperial Fists, leading the newly- created Black Templars). In that case, some captains, commanders and vet sgts may initially form the leadership of that chapter until its own recruits gain the experience and time to do it themselves. These select marines would wear the armor and heraldry of the new chapter, not their old one. However, I have not heard of 2 separate chapters who are both weakened to join together. They either recover, or perish.
