This is something that is causing me real confusion. How are we supposed to know when an upgrade bought for a model can simply be noted on paper and when it has to be represented physically on the model?
A few of the ones I`ve noticed are Discs of Tzeentch (I`ve found a model for this on the GW site, but it`s not a 40K), Deamon Icons, Spikey Bits etc etc etc... How do we know which is which?

For a GW tourny it ALWAYS has to be represented on the model itself, if you're just playing w/ friends then paper is fine

Unfortunately, all of your examples aren't even that hard to actually model. Disc of Tzeentch? If you can't think of anything else, take a circular monster base, paint it pink, and tell the other player what it is. Icons of all the gods of Chaos are readily available, and spiky bits(??) just take some pins and glue them to the model's arms.

IMHO, it is better to have someone ask "what the hell is that pink thing your lord is standing on and why did you convert your Khorne champ so that he makes you bleed" than to have that awkward scene where you tell the other player that, yes, you really do have a Disc of Tzeentch, so you really do get that 12 inch move...

Master-crafted weapons should look different from regular weapons. A character with a Mark of Chaos should represent their god's characteristics. The only things I don't think you should have to show are stuff like veteran abilities, or something small enough that it can easily be concealed (grenades). I have a character on bike with a power weapon, but he isn't holding one. Solution? I added a somewhat-fancy scabbard and said the sword was in that.

For one-off games, it doesn't matter. But for "official" games, WYSIWYG.

All your examples are easy to model. There are things that cannot possible be represented though, like veteran skills and psychic powers (read something about the latter and WYSIWYG a while a go).
Daemonic Gifts that are somewhat tricky to model are Resilience, Essence (impossible), Fire, Aura (again impossible), things like that.

Please never mind my examples. Really, how do we know whether or not we`re supposed to try and model any adility listed in the Chaos Codex? Just in general y`know? And without have to hack off arms and stuff.

well you just gotta take a little bit of logic with a dash of common sense, things such as Daemonic flight must be represented but something like Daemonic speed doesnt. but your best bet is to take your list to GW and ask an employee what really needs to be represented

Certain things have a description in the wargear part. For instance for Speed they mention additional limbs, though that's kinda tough modelling on a CSM. Just use some powerful looking legs from another model for those (I'd model it if I used it).
