Take a tactical havoc squad with all meltaguns or plasma guns. I prefer Meltas in Rhinos more so than Plasma but its all a matter of preference. If you are playing Death guard you can't take an obliterator and if you do it's elite not heavy. But if you are just playing Nurgle non deathguard you can just not give your havocs the mark and give them lascannons. Also I'd give the pred lascannon sponsons over the heavy bolters for anti tank but this makes it more specialized. Just some food for thought, it's all you.
Wraithlords are easily brought down by a large pack of Plague bearers(14), their daemonic venom ability will always wound on a 4+, plus they have 2 attacks(3 if they charge) and a 5+ INV save.
Dreads are a good choice to destroy Tanks and are the only units which can carry heavy weapons, i have 3 in my army all with missile launchers and either plasma cannons or las cannons.
A havoc squad with 4 Meltas are also good, add 3 marines with bolters as cannon fodder and upgrade a marine to Aspiring champion and give him powerfist or melta bombs and a combi bolter-melta. The whole squad could then be given either tank hunter vet. skill or infiltrate to get them in close to the tanks.
I agree, plaguebearers or Possessed with talons are your best bet, although a Dred could easily do just as well in CC or at range. If tanks really are your biggest problem then meltaguns are handy to have around, especially on infiltrators. Also, give the Champions in your squads power gloves( this is pretty handy anyway) just in case any armour wanders too close.
Also, I don't know if you already know this, but all-Nurgle armies cannot use Obliterators... shame I know...
