A friend of mine wants to start a khorne worshipping Black Legion army, but he wants to know if he's allowed to use the regular CSM models as khorne marked soldiers as opposed to buying Khorne Beserkers as he feels that they look too much like world eaters.

maybe paint one of the shoulder pads in red gore to symbolise the khornate alligiance

I don't see any necessity to represent post heresy World Eater colours.
It's states plain as day that many warriors feel the call towards Khorne. I think he can paint them any colour he wants.
I DO however, think that Khornate icons and the like are important.. Naturally, if you want to use chain axes then they'll need those modelled too.
The easiest way would be to stick a great big Khornate banner on the aspiring champion. Everyone in a 2 mile radius can see they're khornate.
