The simplest thing to do would be to get an arm with a bolt pistol. Remove the pistol and replace it with a boltgun. Another option is to get the sergeant boltgun that comes in the devastator box set or order a couple of them online.

a chainsword arm (opposite the fist of course) works as well, just trim at the crossguard

You could convert your sarg to look like the sternguard sarg in chirality's avatar.

Devastator Sgt. has a guilded Boltgun. Attaching a Pic of mine.

But any bolt pistol arm can be, cut right over the hand on the grip of the pistol, and the grip of the boltgum removed, they fit really nicely.

Bolter arms will work as well. I made a number of my basic tactical marines with a bolter in 1 hand and a bolt pistol in the other. You just have to trim the shoulder attachment point on the arm so that it mounts at an angle that the arm comes straight out, instead of close to the body. The shoulder pad over top of this modification covers any of the cutting.

I would only worry about it if you want the look. The model comes with standard gear of bolter pistol and bolt gun. The powerfist is an upgrade to replace chainsword, so it must be modeled. But the standard gear doesn't "have" to be modeled, only if you want to. I don't bother with them.
Age is a state of mind, and since I have no mind, I don't age.
