This might belong in the rules forum, but I think it would be better off here. Anyways...
Am I reading the rule under the heading Space Marine Characters on pg 21 of the sm codex correctly. It says that "Lirarians and chaplains may be attached to another character's Command Squad. Only one character of a given type may be attached to a single command squad."
This makes it sound like a commanders command squad could possibly have one librarian and one chaplain added to it. This would mean that 2 commanders with two chaplains and two librarians added on would be legal, and would total 6 HQ units for a marine hq. Thats pretty impressive (not that anyone would ever do it...).
Am I reading this correctly? And if it has been discussed befoe, I'm sorry, but I didn't get any results when I searched for it.

Though I imagine that'd be a bit tricky to find with search.

You wouldn't have 6 HQ units, you would only have 2 units each with 3 independent characters. It is an expensive way to go and makes a tempting target as well. You can use the following combos...
Commander with Chaplain and/or Librarian
Chaplain with another Chaplain and/or Librarian
Librarian with Chaplain and/or another Librarian
Remember too that you have to take a Command squad to take the additional characters.
Man this just screams some crazy hand to hand units with such combinations. Only drawbacks would be the pointsinks, which could be bad.

Sorry, Lord Malachi (nice avatar btw:yes: :yes: ), I didn't mean to say "units", just "models" I guess. And yea It would come to over 500 pts most of the time( for just one unit with three characters), with out upgrading the serg or comp champ or apothecary or standard bearer.
I haven't tried one of those squads yet, but they would be fun to try in a high points game (2500+).
I've seen some DA players try it out.
The Chaplain(for the rerolls) combined with a Librarian(for extra attacks on the Captain) with Might of Heroes and a Captain(For a Strength 6 power weapons) with the Sword of Secrets for supreme goofiness.
While they kick ass in hth, they almost kick too much ass, as they typically will slaughter any opposing squad on the charge, leaving them stranded and plasma gun bait.
