I searched and I couldnt find one of these, but if there is one please correct me if I am wrong.
I just wanted to gather a list of all of peoples custom Space Marine Chapters, so eat your hearts out and give any information you want, but here is a guideline:

The Shadow Hawks are a successor chapter of the Raven Guard.
Specialised in sabotage, infiltration and assassination, the Hawks live on the planet of Opacus Mundos (means shadowy world in latin), a world which has in its upper atmosphere a fog that does not allow sunlight to penetrate fully, the thermal warmth is capable of penetrating the clouds. This creates world is subjected to a everlasting shade on the dayside and pitch black on the other. People have settled in colonies which can be found as radiant blobs on the planets surface. The Shadow Hawks have situated their Fortress Monastery far away from the cities in an area of inhospitable terrain consisting of rocky outlets. Initiates must locate and reach the Monastery in their own without any major help, to begin their training as a neophyte. The Shadow Hawks train regularly in the dark areaâ€™s surrounding their Monastery and are renowned for their navigation and fighting techniques at night.
Their Chapter Master is Vortar Kelusa. He has led the Hawks through many campaigns and has had perfected the techniques of fighting at night conditions to an unparalleled level.
They use special â€śshadowâ€? groups of veterans to sneak up the enemy and cause havoc or to assassinate the enemy leader.
They make great use of specialised groups of Devastators who have mastered to sneak up the enemy to pick the best fire-location.
Tactical squads are jack of all trades capable of infiltrating near the enemy or to be used as a diversion attack force.
Assault teams are used as killing blow units, used to destroy units broken up by devastator or tactical squad fire. They are also used as rapid response units, dropping from thunderhawks on top of the enemy.
The Shadow Hawks way lies in stealth, so they donâ€™t have a battle-cry as such. They do have a motto which is taught as the foundation of their warfare:

The standard approach is to start with a raid of a â€śshadowâ€? group to break enemy morale and eliminate any major threats.
After this a sudden attack will take place from tactical squads who open wire from a distance and slowly advance. When the enemy is about to engage the Tactical squads devastator and scouts will fire at the enemy confusing them, in this chaos the â€śshadowâ€? groups and assault squads arrive to finish of the enemy.
Other way of attack is that the entire army suddenly spooks up from the darkness and launches a devastating strike, mostly from up close, to be certain that the mission objectives are accomplished.
The hawks do not use many drop pods, since the course canâ€™t be changed after release. The chapter does have a lot of thunderhawks since these can deliver troops fast and is capable to relocate and change directions if better targets are located.
Also the fact that thunderhawk landings leave almost no mark behind, in stet of a half demolished pod is important since there is no place from where the enemy can track the troops or even know of their presence.

Chapter Name: Red Devils (Second founding of Imperial Fists)
Home world: Trafford, a tome world home to thousands of manuscripts and records on the Imperium of Man. Located in the Segmentum Ultima, the Red Devils stand guard over the northern area of this region. The Red Devils Fortress Monastery is nestled between two immense mountains on the northern hemisphere. The planet’s only inhabitants are the Red Devils and the servitors and scribes who keep the records.
Chapter Master: Ernest Mangnall (first Manchester United Manager)
Traits: Heed the Wisdom of the Ancients, Honour your Wargear
Disadvantages: Eye to Eye
Paint Scheme: Scab red armor with blood red highlights around shoulder plate and knee pads.
Fluff: The Red Devils are a second founding chapter of the Imperial Fists. The vast libraries on Trafford contain stories of honor and valor of warriors past, and the Red Devils are strongly devoted to the memory of their fallen comrades. The chapter has a solid base of veteran warriors, many entombed in Dreadnaughts, who lead by example in battle. The Red Devils also have a number of excellent psykers, conjuring the fury of the ancients and leading the chapter tactically. Initiates begin as scouts, and when they are given their power armor, they are also assigned to one of the chapters elite devastator units.
Doctrine: The Red Devils, when deployed, quickly establish good positions for firing. The Chapter is excellent at defending positions and giving good mobile fire support, but lacks strength in assault. The companies Venerable Dreadnaughts serve as good mobile fire power, while the elite devastators hunker down and just shoot anything and everything. The Red Devils also have a number of excellent psykers, conjuring the fury of the ancients and leading the chapter tactically.

Still working out the details. What I've got so far:
Arrival: Was originally a company of the Ultramarines. Sent to some colony world to investigate a possible unearthing of a chaos artifact. They arrive at almost the same time as a chaos fleet. The resulting battle destroyed both fleets. The remaining marines crashed on the planet in one of the smaller battle barges (a large one crashing probably would have destroyed the planet). All other ships were destroyed.
Isolation: Turns out the planet has a strange property in the fact that it is abundant in psychic crystals. These crystals make a background noise that makes astropathic communication from the surface impossible. With the crash of the battle barge, most Thunderhawks and Shuttles were destroyed, what remained were not space worthy. The SM company became stranded on the planet, unable to achieve orbit and with only two or three small colonies to support them in exchange for the SM's protection. In the meantime the imperium believed the colony to have been destroyed and all forces lost. After a couple hundred years, the bureaucracy decides to send an exploratory force to investigate. But as things go, it takes several hundred more years for the force to arrive.
Orion: Soon after crashing onto the colony planet a techmarine by the name of Orion began to command much presence and through insight and valor eventually became company commander. Using the techpriests secrets he began to repair what remained of their vehicles. Believing themselves abandoned imperium and with no sight of the Astronimcon, many of the battle brothers began to worship the Machine God. Through much work and the teachings of Orion, many of the vehicles were repaired to working order. However things like the Land Speeders were beyond their ability to fix without access to the anti-grav materials used to make them hover. Upon rediscovery by the Imperium, the exploratory force found that the brothers had formed a very tight cult that was still loyal to the Emperor, but had no need or desire for outsiders.
So there's the basic fluff, I came up with most of it this afternoon. The reasoning for isolation is a little flimsy, but I didn't want to say "Just another warp storm". I chose to use an Ultramarines company as the founding because of the colors. I just really like Ultramarine blue. I also use silver highlights instead of gold.
I'm not actually that good of a writer, so it's kinda flimsy in places. But I'd love to hear peoples opinion of this.
EDIT: Forgot to mention one thing about my paint scheme. I was going to make the vehicles look like they're being patched. Put extra plates on that look like they're bolted patches. Also make it look like those cars that have been to the body shop, but haven't been painted yet with panels that are still gray primer.

Chapters founding date is unknown but is thought to be in the late 32nd millennium or early 33rd.
Unknown heritage, possibly Ultramarines or successor chapter. Slight mutations include a reduction in the efficency of the omophagea rendering the marines unable to gain knowledge through ingesting flesh. The chapter also suffers from a shortened lifespan compared to other chapters, marines over 200 years are uncommon. This has resulted in an increased specialization, especially notable is the almost complete absence of assault squads in favour of squads armed with bolters and close combat weapons able to perform multiple tasks.
The chapter is undoubtly very old, it's own records stretch back to the 35th millennium and other imperial organizations indicates that the chapter may have been founded as early as the 32nd millennium. Even so the reduced lifespan of the marines, most evident among it's veterans, make it a young chapter in practical experience. This has forced the individual marines to specialize in their assigned weapon from an early age, usually during their scout training, to achieve full proficiency. When the marines are divided into squads after their training there are very few marines trained in the use of close combat weapons, so the chapters assault squads are commonly replaced by experienced marines who have mastered the bolter fully and is able to use it in a close combat situation.
Even though individual marines compare favourably to other chapters due to their specialized training the officers are comparatively young and inexperienced. This has led to the chapter preferring to put themselves under the command of other chapter commanders when possible and occasionally even experienced imperial guard officers. They also regularly send small forces of marines to aid other imperial organizations such as the deathwatch and Adeptus Mechanicus explorators where their lack of strategic skill is unimportant and their ability to cooperate is valued and reinforced.
The chapters beliefs are similar to other chapters of the Adeptus Astartes. Since they do not know of their own primarch they venerate each equally and observe holy days for the ones who have remained loyal as well as celebrating the day of the Emperors ascension.

Varies, one is usually selected before the battle by the chaplains. Alternatively they may use their allies battle cry if appropriate.
