Yep this probably is the aformentioned tank.
Though as far as the chassis goes, it looks like it may even be a chimera or an older IG leman russ model.

That is a tank from "Lock, Stock and 40.000 Smoking Barrels" by Kevan Downey UK GD 2000
EDIT: The Hull is made of Plasticard and the tracksections comes from a Russ. The Gun seems to be using both a Russ' and plastic tubing.

Hey i am a big history buff and i have seen this tank to and love it and it is not a true GW kit but it is actually based off a real life tank and could easily be converted! here is the can in question!
This is the German WW2 STUG IV infantry assault vehicle and i think you can see where the converter got their inspiriation! Hope this helps! Btw there are plenty of model kits out there that are this exact tank! With some IG bits this could definatley be turned into an awsome looking 40k tank!
There is pictures of this in both the old city fight book from a couple of different angles showing the two tanks.
