umm just a quick glance... empre/bretonian standard bearer arm with a VC zombie/skeleton scythe attached. Ork shoulder bads and maybe an eldar sword? The upper part of the arm looks to be from the cadian sprue. I can't place the heads on the backpack however....skaven?
no offense but he looks extremely stumpy for an inquisitor.. but otherwise good convert.. i'm no good with bits but the backpack is from the chais tank sprue..
I didn't want him to look big, as he is to be fielded alongside Space Marines, and I hate it when regular humans are taller/bulkier than Marines.

I do agree with thefallenone on the stumpyness, and I do understand how you don't want him to be as big as the SMs too. I think you'd come a long way if you gave him another head, try a human head from the WHFB-range, it'd make him look a bit more balanced. You might also want to take his arms of a bit as they too seem to be a bit on the big side.
But all in all it's a nice collection of bits and I bet he'll look great on the table.
Oh and the backpackheads are from the Chaos Vehicle sprue.
His sword arm is from the marauder sprue, and he`s got a SM torso. The skull on his base is also from the chaos vehicle sprue..
Backpack is from the Grey Hunters sprue
Actually guys, I'm pretty sure that's a choas warrior sword, legs the bottom half of a brettonian man at arms? only thing that stumps me is the left arm, It's definately not empire, or brettonian I don't think, at a guess marauder?
Oh and yes they are from the choas upgrade sprue, as I used them to make the deamonhammer for one of my inquisitors....
