How do you use them? How many per unit? Are the weapon and command options available worth the points?
I state that I think Blood Knights are the absolute best cavalry in Warhammer. They are the hardest hitting, close to the hardest to kill (with Regen BS and you cannot beat the vamps ability to bring back losses. Albeit only one per IoN casting.
Are they worth the points? Not in the least. For both the points cost for the game and the ungodly price that GW has these models. $90!? For five of them? Ya, I think that is one of the most overpriced model they have. That makes us all convert our Bret/Empire/any other horse out there.
I would honestly take Blood Knights if I am going into a friendly battle against my buddies where we know ahead of time what army we will play. I think you can easily make a better list for a tourney without using them. They are a points sink and become a Death Star unit. And we ALL know what happened to the Death Star. Twice.
But, if you really want to know what I would take, it would be full command at maybe six to a single unit. This would be with the previously mentioned BSB with the Regen Banner. The magic standard would be Royal banner of Strigos. I would use them on my flank or where ever the weak point would be against my enemy. I would use them to punch through whatever it hit. Theoretically I could use other units to tie up the tougher enemy portions, but with six Blood Knights, I would be lucky to afford three ghoul units. LOL
Like I said, I don't use them because I enjoy the variety I can get with the rest of the army. But, I would never say they are worthless or useless. It just doesn't fit my play style. But, if I face off against another VC army that used them, they would scare the hell out of me. They are not a unit to mess with.
As far as the Kastellan, I would take the +1 extra attack for him so I can get the 4/5 attacks rather than 3/4. At strength five, I just all but ignore the killing blow from him. Figure if I'm wounding anyways, it should be enough to kill what my intended target was.
And to add to it, if this was a list I would take against a buddy, knowing ahead of time what to worry about, I would consider taking the Blood Keep banner for the unit to give myself that 4+ ward save against ranged attacks.
So there is my two cents...
Well for a start, never put your lord in this unit.
I say either 4 naked knights or 5 with banner of blood keep.
Also you need a bat unit and some dogs to prevent them being led around the table.
(BTW I've only just started painting my blood knight unit even though I bought them on release date).
I've stuck lesser characters into a regiment of 5 of them. 5 Blood Knights plus a vampire with Infinite Hatred, Dread Knights, and the Blood Drinker are a dangerous little unit. Of course, you need something to keep them from running around on you. And that's usually what stops me from taking them. Not only are you paying gobs of points for them, you're also paying points on wolves, or bats, and the ability to raise them quickly. It's no fun. I'd rather run Wraiths, as my army is already backed with enough magic to stop most of the spells which would otherwise target my ethereals.
5 of them, sometimes with standard, sometimes not. If with a standard I usually opt for Royal Banner of Strigois. As they're frenzied already and must pursue Hatred adds no additional disadvantages, so the re-rolls are a pure gain. Alot of attacks means the banner is excellent value. There are times though when protection is more worthwhile and the Flag of the Blood Keep is probably the better choice. This depends on your opponent and your army lists ability to deal with incoming shooting threats. Even so the RBoS is my choice most of the time.
I strongly advice against a Kastellan. Having to challange may very well make your costly unit near useless VS lone characters on mounts where the rest of the unit is just going to stand around doing nothing. Your 'normal' Blood Knight can do this anyways should you choose to accept a challenge. So on that note I would avoid the Kastellan as he may very well equal the switft destruction of the unit. Getting stuck with a powerful character and to suffer from the effects of overkill... The crumbling afterwards will hurt alot given the high cost per model.

Although they are expensive I dont think many people who shy away from them know how strong they really are.
Lets say I got 5 blood knights and a Kastellan. The unit is always Frenzied so instead of 2 attacks each they get 3.
So 3 Attacks x 5 = 15 Attacks, +1 for Kastellan = 16 Attacks (Str 7 on a charge)
Nightmares get 1 attack, with Frenzy makes 2 (the whole unit gets frenzy)
so 5 x 2 = 10 Attacks (str 4)
Basically there is few things that have the solid hitting power to match these guys. If fielded in a smaller battle (2000 or less) its likely that they will be the strongest unit on the field and a headache for your opponent to deal with.
I used the Blood knights this weekened and I remember my friend laughing when my Kastellan challenged his Tomb Prince, but with a Str 7 charge my Kastellan rolled over him without any trouble (sadly he died to a curse immediately after but the unit itself continued to be a big threat all game.
A Vamp with a Bloodrinker sword is a nice addition, too. Bring a single Blood Knight back from the great beyond with it and it has paid for itself.

I usually Run 5 Knights with full command, Royal Standard of Strigos, Champ has Baliefire Pike and BSB with Regen banner, ok the unit is 670pts... a very literal death star though, almost nothing in the game can surivive a charge from these guys, and i have survived Bret lance formation charges with these guys, and the unit champ on the charge with hatred and str 7 will flatten most characters, even lords (just not DE stupid pendant ) oh and about price i bought them on release so it only cost me£40 which is roughly $60
i have seen this unit charge the front of my opponents "anvil" unit, and out the other side enough times to know they are worth every point
Fantasy : Empire, Chaos Warriors, Chaos Daemons, Vampire Counts, Beastmen, High Elves, Lizardmen( but i have opinions on every army just not the money)
40k: Black Templars, Imperial Guard, Dark Eldar, Tau (thinking of Eldar)
i've only taken them once up against a dwarf army and they hauled all kinds of butt. i had 5 with full command with the bloodkeep flag. what made them extra crazy was having not only the bsb but the lord in there aswell. the bsb had the drakenhof banner and dreadknight and walking death, my lord had redfury, walkingdeath and dreadknight. i also gave the lord the dread lance so he had 4 s7 attacks on the charge that automaticly hit, and may as well have automaticly wounded. neadless to say each unit they hit died before my nightmares could do anything. to keep them in line i just kept two direwolf units in front that were moved when i needed them to. it was wonderful, they top my other deathstar choice of 24 GG with GW, i will most definatly use them again (once i actually get the models).
