Commissar Yarrick - He's Commissar cool and can ride with the vets. He can also keep an eye on the psyker.

Primaris Psyker - Because every Vet squad wants a Str 6 AP5 Assault 2d6 weapon.

This list is my first fully mechanized list, so any feedback is welcome.

The working idea is that Yarrick and the Psyker will ride with the demolitions squad in a Vendetta, as will the Vets with flamers. They will support the previously infiltrated Stormtrooper squads. The Demolishers will bring up the rear sowing havoc as they advance with the remaining mechanized vet squads. This list tries to hit as hard as possible on turn one, and mop up the fragmented pieces, before rapidly redeploying to grab the table objectives.

From a fluff point of view any force led by Yarrick is going to get ground down to the hardest of the hardcore, hence the elite infantry with the nice toys.

Yarrick and his abilities only make sense in an infantry force that's geared for close combat, and then only in a game over 2K. Strakken (Counts-as Yarrick?) would be a better choice for what you want. But frankly, I'd just go with a CCS in a Chimera and be done with it. With 3 meltas it only comes to 135 pts. That leaves 130 pts if you nix the Primaris.

Three Stormie squads is overkill. Go with 2 instead and buy them Chimeras so they can scout or outflank meaningfully. Stormies in a Chimera can scout-rush your opponent and put anything further than 4" from his table edge in short range of their meltas before he even moves. Or you can outflank 2 Chimeras and kill anything you like. Finally, you can still deep-strike the Stormies and use their rides for the Vendetta Vets, or just run them forward empty to block with.
