I just finished building a nurgle lord conversion, and I thought it might give you some ideas.
I'll start by giving a parts list:

The idea for this conversion is largely based around the chaos lord of nurgle pictured on the last page of the codex, also some ideas I used come from or are based on the tips from 'Repent!'.
The conversion is based around the body of the impy marine capt. I used a razor saw to carefully remove the models left shoulder pad (sword hand) head, sword and both lower legs. The legs were cut, using the bulge of the exoskeleton as guide. Part of the original tabard must be cut off.
Next I removed the lower legs from the Chaos Term, again using the bulge in the exoskeleton, just above the knee as a guide.
To attach the legs I put in a piece of the 1 mm card that the end of the upper legs to help give the model some more height. These were carefully trimmed, after being glued into place, to match the exoskeleton tubes that run down the upper leg. These were then pinned and glued.
After the glue set, I got to work on building up a base to recreate the section of tabard I had to remove.
While the first go of green stuff se I got to work on the arms. SO replace the shoulder pad I'd cut off of the base model I just used the shoulder from a chaos term power fist. I cut the power fist from the shoulder- and made sure to get all the stuff, even under the tiny overhang that the shoulder piece has. Once that was removed and cleaned I sanded the inside (where it would meet the body) nice and flat. I attached a piece of the 1 mm card to that inner portion of the shoulder to give it some more mass. This card was also carefully trimmed after it was in place to match the shape of the pad.
The procedure for the right shoulder was basically the same. I removed the shoulder pad from the reaper arm, and beefed it up.
Now I had a model with a left arm two shoulders and two legs - I needed a right arm. After a couple failed attempts I settled on using the claw from the new raptor. The arm was removed from it's shoulder and a 1mm piece of card was added above the elbow so the slightly smaller scale arm wouldn’t be sucked into the mass of the model. Once the plastic was trimmed and sculpted a bit it was pinned into place.
Now I thought it was time to fit the head. If you don't have a Dremel, this might make for trouble. I used a cutting bit to scoop out area where the head was to sit, since when I sawed of the original head, it just left this flat area that forced the new head to stick out too much. Once a cavity was made for the new head it was glued into place, and a little green stuff was used to fill in some holes. I also used the dremel to remove the term pendant on the chest. I just removed it and reshaped the tabard a bit so it looked like nothing was there.
Now I finished the resculpt of the new tabard and made a Nurgle Icon for the right shoulder and left those to dry before moving on. I also built of the 'collar' for the amour a bit - all this was done in green stuff
At this point, after the green stuff had fully set I decided that the model was to clean. It's Nurgle after all... SO I set about beating it up, chipping amour making holes in the tabard, snipping off a finger of the claw etc.
I was nearing the end, but I still needed a weapon. So I added the left shoulder pad and got the weapon ready. I used the scythe from the WFB Nurgle lord to make a man reaper. The hand of the WFB Nurgle lord was cut out and the two halves of the scythe were *carefully* drilled. I drilled a holed all the way through the left hand of the model and then ran one piece of pin through to attach the two part of the scythe to each other and the hand.
Last thing to do was the details. I snipped the two front spikes the each of the trophy racks and super glued them to the top of the model. I also cut down the banner pole so it just cleared the notch in the back of the model. Once the super glue was dry I reinforced where I could with green stuff. I hope this won't be too prone to breaking.
Last thing was to attach that right arm and stick the whole thing to a base.
